The Connect for Health service operates across 13 GP surgeries throughout Ipswich. The service is designed to assist local people (aged 18 and over) tackle a variety of non-medical challenges.
Connect for Health social prescribing programme
What is social prescribing and how does it work?
Social prescribing aims to improve an individual’s wellbeing by recognising that their overall health can be negatively affected by a range of non-medical factors, e.g. social, economic and environmental. Support for many of these challenges is often available from local Voluntary, Community or Social Enterprise (VCSE) groups, if only people knew how to access them.
Our Connect for Health team works within Ipswich to improve the lives of those who live there. The core objectives are to:
- Empower individuals to tackle and solve the problems they face.
- Improve health, wellbeing and resilience.
- Reduce social isolation and loneliness.
- Increase levels of confidence, self-esteem and decision-making control.
- Facilitate and improve access to essential services.
Whether you access the Connect for Health service directly or are referred to us by a health/social care provider or a local public or voluntary sector organisation, you will be assigned to a trained Connect for Health Adviser who will work with you to identify the problems you are facing and establish what is important to you.
With the support and guidance of a Connect for Health Adviser, you will develop a personalised action plan to find the solution that is right for you. This may range from helping you to access a variety of national, local or community services; writing letters; assisting with phone calls; connecting you to specialist help; or introducing you to community groups, clubs or projects that are of interest to you.
How do I connect?
Complete the online referral form which will be submitted to our Connect for Health team, and you will be placed in contact with your Connect for Health Adviser.
Complete the third-party referral form if you are a health, social care or other professional contacting us on someone else’s behalf.
Ask your GP surgery to refer you. In many cases, your clinician may actually suggest an appointment with one of our Advisers.
