Contract:              Initially 12 months Fixed Term contract

Salary:                   £20,000 (pro-rata)

Hours:                    37 hours a week


An exciting opportunity has arisen for a PCN Social Prescriber to join Citizens Advice Ipswich.  This role is part of a program of transformation to develop a new model of care which addresses our ambition to deliver person-centred, co-ordinated care across the Ipswich PCNs. Social Prescribers play an important role within the PCN as they promote integration across the wider health and care system.

Social Prescribing supports patients to live their best lives by working holistically, focusing on what matters to them and connecting them to appropriate services and community resources. It provides general practice with an effective referral route for people whose health cannot be improved by clinical solutions alone.

The PCN Social Prescriber will work as a key part of the PCN multi-disciplinary team. They will help the PCN to strengthen community and personal resilience and reduce health inequalities by increasing patient involvement in their local communities.

The Social Prescriber will assess how far a patient’s health and wellbeing needs can be met by services and opportunities available in the community and signpost patients appropriately. Patients will be empowered and supported to take control of their health and wellbeing through appointments with Social Prescribers.

To be successful in this role you will be a confident, self-driven and self-motivated individual with recent experience as a trained adviser or can undertake intensive training to reach the required standard within minimum timescales.  You will have excellent communication skills and be able to self-manage your workload and time.  You will have a passion for making a difference by providing a top-quality service for the people of this PCN.
